- Open the "Printers" diaglogue box in Windows.
- From the menubar select "File" -> "Server Properties"
- Check "Create a new form" and name it 36x24.
- Make the Width 36.00 and Height 24.00 (this is opposite what the Arch D form specifies)
- Hit "Save Form"
- Do the same for a 42x30 inch sheet.
- Close the Server Properties box.
- Both sheet sizes should now be available for use on any printer, including CutePDF. (You may need to restart AutoCAD if it is running)

Creating New Sheets (forms) for CutePDF
Not related to Revit, but I need someplace to write these instructions down. This is so that you can create pdfs from AutoCAD using the CutePDF plotter that are formatted to the correct orientation.
